Pact Team offers many ways to access health care, including personal visits with your primary care provider to visits with other members of your care team.
Home health care refers to medical and non-medical services provided in a patient’s home to treat an illness, injury, or condition. It aims to help individuals recover, manage their health, and maintain independence in a comfortable and familiar environment.
