PACT is a partnership between a patient and their care team. The PACT model is a team-based model of care that focuses on wellness and disease prevention. The care team considers all aspects of health, with an emphasis on prevention and health promotion. Pact Team offers many ways to access health care, including personal visits with your primary care provider to visits with other members of your care team.

Home health care refers to medical and non-medical services provided in a patient’s home to treat an illness, injury, or condition. It aims to help individuals recover, manage their health, and maintain independence in a comfortable and familiar environment.

As the population of older adults in Nigeria continues to grow, the demand for healthcare services that enable them to age in place is becoming increasingly important.

While there are numerous factors to consider for health at home programs and the landscape is evolving, it is important to remember why these programs exist. There is value to healing in the comfort of one’s own home.
